Consent

I authorise staff and representatives of Ballpark Entertainment Pty Ltd (Perth Wildcats) to act for me according to their best judgment in any emergency requiring medical attention and will not hold them liable in any manner whatsoever for any injury, illness, death or detriment that my child may suffer
 
I agree that in consideration of the Perth Wildcats, permitting my child to participate in all activities I agree to indemnify them against all and any action, suit, claim, cause of action or proceedings my child or any person acting on behalf of my child may take against them.
 
I agree for the Perth Wildcats to use my child's image photo or video in all media formats unidentified for its own use or for use by its associated partners.
